I want to take advantage of Tribal Government Institute's procurement assistance counseling. I understand that this assistance is free of charge. I agree to cooperate should I be selected to participate in surveys designed to evaluate TGI's services. I authorize TGI to furnish relevant information to the assigned management counselor, although I expect this information to be held in strict confidence by him/her.

I further understand that TGI's counselors have agreed not to: (1) recommend goods or services from sources in which he/she has an interest and (2) that he/she will not accept fees or commissions from this counseling relationship. In consideration of TGI furnishing management or procurement assistance, I waive all claims against TGI personnel or counselors arising from this assistance.
